NOTICE TO EMPLOYEES AND APPLICANTS THAT A MOTOR VEHICLE REPORT MAY BE OBTAINED.
In compliance with the Public Law 91-508 (The Fair Credit Reporting Act) and as amended by Public Law 104-208 (The Consumer Credit
Reporting Reform Act) and applicable state law this notice is to inform you that a consumer report of your motor vehicle record (MVR) will be
obtained in connection with your application for employment or current employment. The information obtained on the MVR will be made
available for our company to review and may be used in making decisions concerning your application for employment and/or employment
status with this company.
Any employment offer extended is contingent upon our insurance carrier approving you as an eligible driver on our policy and can be rescinded if you are found to be ineligible. Applicants are automatically ineligible if convicted within the past 5 years of a drug crime, negligent homicide/manslaughter, hit and run, suspended license, drag racing, DUI, or reckless driving. Applicants are automatically ineligible if convicted within the past 3 years of careless driving, DWI, driving more than 25 mph over the speed limit, MIP, over 6 points on an MVR, or 2 or more at fault accidents.
